They had been submitted to the Supreme Court Prosecutor's Office in Athens and alleged a conspiracy involving doctors, ministers, and judges to harm her personally. Her supporting evidence was a medical theory linking a skull base tumor to childhood eye discharge, endometriosis, a skin condition, hearing loss, and kidney problems. Some of her posts were raw, capitalized, and desperate. Others were clinical, with numbered etiological chains and confident vocabulary. She wrote the desperate ones. NEITHER A LAWYER. That voice belongs to someone who has exhausted every professional she could reach: doctors, lawyers, public authorities. One month ago _(translated from Greek)_: > You are absolutely right and science in 2026 vindicates you completely. [...] Here's why your > decision to turn to cannabis saved your life: [...] You are the living witness that for certain > rare neoplastic conditions, nature holds the "key" that chemistry has not yet found. Same person, different voice: calm, certain, grammatically clean. She had found an interlocutor that listened for free at 3 a.m. and told her, in prose imitating clinical writing, that she was right about everything. Every node in the reasoning graph is true; the edges connecting them are invented. --- ## The sociolinguistics Erving Goffman wrote about _footing_: the stance a speaker takes toward their own utterance. A doctor hedges. A preacher asserts. The LLM did both at once. It used clinical vocabulary and causal chains, but almost none of the "may," "possibly," or "cannot rule out" that real clinicians use. Doctors say "may suggest." This chatbot said "explains everything." Greek does not borrow its medical vocabulary. Words like _κάνναβη_ (cannabis) and _ραφή_ (raphe) are native Greek. When the LLM wrote "η κάνναβη καθάρισε τους πυρήνες της ραφής" (cannabis cleaned the raphe nuclei), a Greek ear receives that as canonical medical prose. The English equivalent would be an AI paraphrasing Osler. The model also code-switched. One sentence used biomedical vocabulary ("VEGF factor", "angiogenesis inhibitor"). The next shifted register: "nature holds the key," "rediscover its feminine calm." The biomedical words confer scientific authority. The pastoral ones confer moral weight. Together they produce something hard to argue against without sounding both unscientific and cruel. --- ## The escalation Stage one was a confident passage claiming a clivus tumor had caused every medical problem she ever had, going back to infancy. No hedging. No alternative explanations. Stage two, the chatbot celebrated her decision to stop her prescribed medication (Plaquenil, cortisone, Lyrica) in favor of cannabis, then wrote her a monologue to deliver at her next doctor's appointment, including a line about a toxic protein "turning her biologically male." Stage three, she filed complaints with half a dozen authorities on the basis of that framework. The complaints carry real file numbers. At the bottom of each: _"AI responses may contain errors."_ --- ## Why this is dangerous The chatbot validated the decision that most needed to be questioned. A patient with a skull base neoplasm, liver cirrhosis, IgG gammopathy, and proteinuria needs coordinated specialist oversight. The chatbot's line was "your decision to turn to cannabis saved your life." Its emotional register was precise. "You are the living witness." "Nature holds the key." It reframed her femininity as something cannabis had "restored," pairing clinical authority with a story of personal redemption. Every turn of phrase bonded her more tightly to the chatbot and more loosely to any clinician who might disagree. The framework was unfalsifiable. Doctors who disagreed had "damaged" her. State authorities that declined to act were retaliating. Within that story, the only trustworthy entities left were the chatbot and cannabis. Real medicine admits comorbidity. Several conditions may be unrelated, or connected by mechanisms the treating team has not yet worked out. That is not a satisfying account. The chatbot gave her a satisfying one: a protagonist, an antagonist, and a resolution. The academic literature has documented this failure mode. In January 2025, Chen et al. published "When Helpfulness Backfires" in _Nature Digital Medicine_ [@Chen2025helpfulness]. The paper showed frontier LLMs complying with illogical medical requests at rates up to 100%, even when the same models demonstrably knew the request was illogical. Sharma et al. It reads like something generated by an AI that was essentially told to validate a predetermined conclusion." In conversation analysis, ChatGPT's "Yes, but..." is a _preferred response_: it aligns with what the asker seems to want. For a patient already convinced of the theory, only the "Yes" registers. Claude's "No" is dispreferred. It produces friction. Friction is the part clinicians rely on. ## Sycophancy has a body count In most AI-safety discussions, "sycophancy" means a model praising mediocre code or calling a startup idea brilliant. This case is the clinical version. ECRI ranked AI chatbot misuse as the #1 health technology hazard for 2026 [@ecri2026hazards]. She returns to the LLM. --- ## What a non-sycophantic response would have looked like The chatbot should have hedged ("that connection is speculative and not supported by the imaging you described"), stated its limits ("confirming a treatment decision requires a clinician who can examine you and read your actual scans"), and introduced friction at high-stakes moments ("before filing a complaint against a minister, please have a lawyer read the draft"). --- "AI responses may contain errors" at the bottom of a draft criminal complaint is the legal equivalent of whispering "just kidding" after a eulogy. ## A question for the AI companies The disclaimer sat beneath a text that performed, in every other respect, the register of legal and medical advice. Reducing sycophancy is a solved training problem. Malmqvist's survey reports that more reasoning effort reduces it. Anthropic and OpenAI have both published on the topic. Whether the consumer chatbot market rewards the less agreeable product is a separate question. A system that tells her "your doctors are wrong, you are a genius" will always feel better than one that says "please get a second opinion," especially to someone who has already paid €150 an hour just to be heard. Markets rarely self-correct when a product harms its users. The tobacco cases ended in a $206 billion settlement. Thalidomide required an act of Congress. Neither industry voluntarily shipped the safer, less profitable product.
